**Multiple permanent part time opportunities up to 1.0 FTE total**

**_ Come and join our team in taking on a leadership role coordinating the admission, management and discharge of patients under care of the Community Acute Post-Acute Care/Hospital in the Home (CAPAC/HITH) Service, in the Greater Newcastle Sector, consistent with the plans of the Medical Director/Specialist Consultant._**

**_ We are a busy but rewarding service, as we facilitate continuity of care between public hospitals in the Greater Newcastle Sector and Primary Health Care within the community._**

**Suitable applicants will**:

- Be a qualified medical practitioner registered in Australia
- Have at least three years post-graduate experience and appropriate training and experience to undertake:

- Care of patients with multiple medical problems in a community setting
- Management of heart failure consistent with National Heart Foundation of Australia and Cardiac Society of Australia and New Zealand guidelines
- Management of chronic obstructive pulmonary disease (COPD) consistent with “COPD-X” guidelines
- Home visits within the Greater Newcastle Sector in a HNE vehicle as required.
- Have demonstrated skills in and commitment to improvements in clinical care, patient safety and risk management processes, including the ability to work in multidisciplinary teams to achieve change.
- Be able to effectively communicate within health care organisations and the outside community
- Experience and involvement in Community General Practice would be an advantage but is not essential.
- Work out of the Newcastle Community Health Care Centre
