About the role
Maroondah Continuing Care Team is a dynamic multidisciplinary team providing case management and collaborative recovery orientated treatment and care for adults with severe mental illness. We work with consumers, their carers, families and other community agencies to identify and address needs, coordinate care and support consumers in their recovery
The Team Leader is required to provide professional leadership by ensuring that high quality clinical standards are met within the team. The Team Leader is a senior clinician and also carries a clinical caseload, as well as administrative and supervisory responsibilities for the day to day operation of the team.
The Team leader works in partnership with consumers, carers/ families and other service providers in order to provide high quality discipline specific and mental health assessment and consultation services within a recovery orientated framework.
The Team Leader works as part of a multidisciplinary team and is responsible for providing comprehensive assessment, intervention and community linkages for the identified client group for a time-limited episode of care in accordance with established Eastern Health Mental Health Program Standards, Practice Guidelines Model of Care. The position will also take an active role in the ongoing implementation of the Collaborative Recovery approach with the Adult Mental Health Service (AMHS) Continuing Care Team at Eastern Health.
About you
**To be successful in this role you will possess**:
Current registration with AHPRA
Extensive experience in community mental health service delivery
Highly developed interpersonal and leadership experience
A strong commitment to high quality care
The ability to work as part of a diverse team
Excellent communication and organisational skills
