**About the Role**
We are currently seeking a sociable and organised Support Coordinator to join our dynamic team. In this role, you'll play a vital part in supporting NDIS participants, helping them understand and implement funded supports outlined in their plans, and connecting them with appropriate services. **Qualifications**:
- Minimum 1 year of Support Coordination experience
- Understanding of NDIS legislation, price guide, and rules
- Ability to work independently and as part of a team
- High level of confidentiality and sensitivity in managing information
- Relevant territory qualification (highly regarded, but not essential)
- Exceptional interpersonal skills and relationship-building abilities
- Proactive mindset with the ability to work under pressure
- Excellent written and verbal communication skills
- Intermediate proficiency in Microsoft Office
- Current driver's license
**Responsibilities**:
Your role as a Support Coordinator will include:
- Seeking new participants and following up on referrals
- Implementing NDIS plans and assisting participants in choosing preferred options
- Assessing all options for community, mainstream, informal, and provider supports
- Maintaining rapport and building professional relationships with participants and stakeholders
- Reporting to the Senior Support Coordinator and Support Coordination Manager
- Empowering participants to understand, implement, and review their NDIS plans
- Monitoring expenditure and keeping participants informed about expenses
- Actively maintaining professional knowledge in disability services and NDIS guidelines
Pay: $70,000.00 - $80,000.00 per year
