**About the Role**

As a Team Leader within our aged care program, you will be providing support to clients who access our Commonwealth Home Support Program, Veterans Home Care Packages and Home Care Packages in Canberra and surrounds. You will work in partnership with our clients, support workers and various stakeholders to ensure clients have up-to-date care plans reflecting their individual goals and access to services and resources that provide them with holistic care.

This is a permanent full-time role, with some travel required within Canberra and surrounds to meet with the clients and relevant stakeholders. You will be joining a supportive and engaging team who enjoy using their time in the office to work collaboratively and bring the best possible service to the people we support.

**Key Responsibilities**
- Undertake comprehensive consumer assessments, care planning and review
- Develop strategies and/or broker services to address the client needs in consultation with families and communities
- Allocate home care workers and maintain changes
- Maintain regular communication with consumers and their families
- Liaise with community-based agencies and service providers to ensure an integrated and cohesive approach to service delivery

**Skills & Experience**
- Degree or equivalent experience in human services, nursing or related field
- Previous Case Management experience and a passion for working within the Aged Care field
- Current Australian Drivers Licence
- Demonstrated experience of working with older people, including people from culturally and linguistically diverse backgrounds, and understanding of issues facing people with chronic and complex needs.
- Demonstrated ability to work in a flexible and innovative way in responding to consumer need.
- Demonstrated experience in all aspects of case management including comprehensive assessment, planning, provision and evaluation of services for older people, people with dementia and or mental health concerns and their carers and families.
