- The Allied Health Chronic Disease Intermediate Care Services bring together Dietetic, Exercise Physiology, Occupational Therapy, Podiatry, Physiotherapy, Social Work and Case Management services working collaboratively with services from the NALHN Clinical Divisions and Aboriginal Health Services to optimise health outcomes and reduce hospital admissions for patients with Diabetes, Cardiac and Respiratory Diseases, and vulnerable people with other chronic diseases.Service delivery has a strong focus on client centred care, self-management support, behaviour change and motivational interviewing techniques, which means we listen to our clients to learn what their health concerns are, we support them in working out how they think these concerns can be best addressed and work with them in setting their own goals toward achieving this.

We have several Casual positions available across all clinical areas, which may be appointed at either AHP1 or AHP2 classification, determined by level of clinical supervision required and experience.- The Allied Health Professional (AHP1) works under direct clinical supervision and may provide direction and support to allied health assistants and students.- The Allied Health Professional (AHP2) works under limited direct clinical supervision and may provide direction and support to less experienced therapists, allied health assistants and students.- If you think you might be the perfect person to join our team, we encourage you to contact us for more information.
- To be eligible for this role you must hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service, specifically:

- Appropriate Degree or equivalent qualification which entitles registration with the Podiatry Board of Australia.
- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Dietitians Association of Australia (DAA).
- Appropriate Degree or equivalent qualification, usually in human movements, inclusive of the exercise physiology elective and eligible for accreditation from Exercise Sports Science Australia (ESSA).
Appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.- Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Australia.
