**About TCP**:
The largest transitional care facility in Perth’s Northern suburbs provides short term flexible care options for the elderly following a hospital admission. The program is goal orientated and treatment based with the aim to reduce unnecessary extended hospital lengths of stay and premature admission to residential aged care.

**The exciting opportunity**:

- Clinical responsibility for a medium sized facility 20kms North of Perth
- Oversee the delivery of person centred care to short term residents, who stay up to 12 weeks
- Manage, provide clinical leadership, advice and support to Nurses and Carers, creating a culture of best practice
- Ensure the facility is compliant with legislation and accreditation requirements
- Complete clinical data audits, conduct trend analysis and deliver reports
- Identify areas for continuous improvement and develop and implement action plans to address gaps
- Assist to conduct regular admissions and discharges

**About you**:

- An AHPRA Registered Nurse and experience as a Clinical Nurse or Clinical Manager in residential aged care or older adult services
- Sound understanding of how governing legislation applies to meeting accreditation standards
- Demonstrated ability conducting audits and interpreting the results to identify gaps and areas for improvement
- A motivated and adaptable professional who thrives in a fast paced environment
- Dynamic manager who leads by example to inspire staff to achieve best outcomes
