They are looking to add an experience **Care Coordintor** to join their team based in Bundoora. To help deliver the high quality of care that they are reputable for.

**About the Role and your Responsibilities**:
Reporting to the Operations Manager you will play a pivotal role in safeguarding participant's well-being and co-ordination client care needs, including assessment, case management and development of an individualised Care Plan. Employing your professional skills and knowledge, you will assist clients to develop attainable goals and, in turn, coordinate a range of services that support and promote these goals.

Ideally, you will be someone who can maintain a high level of professionalism and work as a dependable member of the team. Some of your responsibilities will include:

- Assisting the clinical management team with compliance documentation and comprehensive assessments
- Providing support and supervision of Care Assistants
- Implement training programs and deliver training to care staff.
- Coordinate the provision of quality person-centred care according to the client's needs, service/care plans and available funding in accordance with a budget.
- Clinical in-home services include a care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, and diabetes.
- Manage client budgets
- Provide prompt and solution-oriented service to clients
- Address and manage client complaints/feedback

**Skills and Qualifications**:

- AHPRA Registered Nurse with current registration status
- Comprehensive understanding of the NDIS framework, policies, and standards
- Demonstrated understanding of the needs of people living in the community
- Experience in coordinating a program for older people or people with a disability in the community and understanding of consumer-directed care
- Excellent documentation skills
- Computer literate and competent in the use of various apps
- Excellent communication skills with the ability to liaise and consult with a wide range of clients, health professionals and team members
- Own registered vehicle and current driver license
- NDISWC Clearance and Police Check
- Right to work in Australia
