The Monash Health Post Acute Care Team is excited to announce a temporary position available for a Clinician (Allied Health Professional or Registered Nurse). This position is full time from 20/3/23 until 14/7/23 to cover Long Service Leave and presents an excellent opportunity for likeminded Clinicians to join a dynamic team that strives for excellence in Service delivery to our clients.

Our aim is to empower and prepare clients to self-manage their health and health care.

Working with clients across the aged spectrum, Monash Health Community provides a continuum of care, supporting clients through all stages of non-acute care from prevention through to rehabilitation. We provide **services t**hat support clients in their homes, community based facilities and dedicated Monash Health Community hubs.

- The role is integral to ensure that referred clients for PAC are triaged, registered and allocated to clinicians in order to co-ordinate services.

**About You**:

- You are experienced, flexible and dynamic Nursing or Allied Health Professional that is looking for a change in career.
- We are a high performing team and we ensure that our client’s journey through Community Health is seamless and timely, it is therefore vital that we have a like-minded Clinician to continue this journey with us

**What You Need**:

- Relevant Qualifications in Allied Health or Nursing Related Field
- Current Victorian Driver’s Licence
- Demonstrated recent experience working in an acute or community setting
- Minimum 5 years post graduate experience
- AHPRA registration or registration with National Registration Authority
- Australian working rights
