**The Opportunity**
Social Connections is seeking a highly motivated people leader, passionate about working with aged and disability community members.
In this role you lead the Morton Social Support team to deliver a range of programs and initiatives which are designed to encourage and promote independence and well-being of frail older people and people living with a disability, whilst providing respite and support to carers. This service is funded by Commonwealth Home Support Program (CHSP) and Home and Community Care Programs for Younger People (HACCPYP).
**Key Responsibilities**
- Lead, supervise and support a team of suitably skilled staff and volunteers in the delivery of Social Support Programs.
- Facilitate the coordination of the program activity planning, implementation and evaluation of Social Support programs in line with Council Policy and Procedures and legislative requirements to achieve a high-quality service.
- Manage incoming referrals and develop, monitor, and review individual service provision plans.
- Coordinate the administrative processes required for client records, financial administration, and contribute to budget and resourcing systems of Community Care by meeting funding and service agreements within allocated budget.

**About you**
**_ To be successful in this role you will have the following:_**
- Tertiary qualification in Health Promotion, Leisure and Health, Community Development or Disability with relevant experience in this field.
- Experience in supporting, leading, developing and mentoring a team to achieve departmental and organisational goals.
- Experience in activity planning, implementation and evaluation of Social Support Groups.
- Current valid First Aid Certificate (Level 2).
- Current valid Victorian Driver’s Licence.
- Must hold or be able to obtain an Employee Working with Children Check.
