Dignicare Health Services Pty Ltd is a privately owned Western Australian company based in Perth operating since March 2022.

Dignicare Health Services offers a variety of NDIS support services to clients with special needs and aging symptoms.

Our team consists of experienced medical professionals and community service workers who offer services to enable clients to live their lives with more independence and ease. Services include group / community-based activities, life skill development and support, personal and household tasks assistance, nursing care, as well as travel/ transport assistance.

We are currently seeking a skilled and experienced Support Worker
- Coordinator/team leader to join us in a full-time capacity. As a team leader, you will be responsible for managing other support workers, planning and executing care plans for clients, administrative tasks as well and ensuring that all activities and care are provided in line with the organizational policy and procedures, OHSC, and Medication administration policies.

In your role, you will work closely with individuals to identify their goals and develop and design support solutions to achieve participant outcomes

**Responsibilities**:

- Lead the team of support workers.
- Establish rapport and develop working relationships with clients and other service providers with an objective to achieve all elements of the care plan.
- Support clients to participate in age-appropriate activities and tasks including employment, in-home support and community access.
- Support clients to access activities and outings in the community.
- Provide direct assistance to each client in accordance with their service plan and as directed.
- Carry out administrative duties.
- Work in accordance with the medication administration policy, work health and safety procedures, and client rights pol cies inclu ing privacy and confidentiality.
- Monitor the safety and well-being of clients, resolve and report any concerns and incident reports to the Manager.
- Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication books as required.
- Participate in the Lifestyle Service Planning process for individual client and monitor outcomes.
- Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.
- In conjunction with ther members of staff, assist in developing, implementing and evaluating training and behaviour program to attain identified individual goals.
- Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
- Assume the role of key support worker for designated clients, including the preparation of a monthly report on client progress.
- Liaise extensively with other staff to ensure overall smooth and consistent running of the service.
- Actively contribute to a team environment through open communication, conducting regular staff meetings planning processes and policy review.
- Provide backup and support to other staff, as necessary.
- Participate in Performance Development meetings to identify ongoing training and support opportunities.
- Contribute to Case reviews with Employment Consultants and Facilitators.
- Carry out their roles and responsibilities as detailed in the relevant policies and processes.

**Qualifications**:

- Minimum Certificate Ill in Disability
- Current National Police Check
- Working with Children Check.
- First Aid certificate and CPR
- NDIS Workers Orientation
- 100 Points of ID

**Skills and Experience**:

- Minimum of 2 years relevant experience
- Previous experience supporting people with disabilities.
- Leadership experience - the ability to lead, delegate, and train others.
- Demonstrated proficient written, verbal, and interpersonal communication skills.
- Demonstrated ability to work with mínimal supervision, and collaboratively as a member of multidisciplinary team.
- Previous administrative experience.
- Ability to effectively prioritize and organize own work/caseload.
- Demonstrated commitment to professional development, quality improvement, and evidence-based practice

**Salary**: $70,000.00 - $90,000.00 per year

Schedule:

- 8 hour shift

**Experience**:
