**What You Will Be Doing**
The role is classified as a grade 2 social worker, community-based RN or Community Development Worker.
Provide an outreach service to people experiencing or at risk of homelessness. This will include supporting clients to connect into relevant health, housing and welfare services from a service coordination framework.
Assist clients to engage with relevant mainstream and specialist service providers as appropriate.
Utilise the Social Model of Health framework and Trauma Informed Practice.
The role is available at 0.9EFT or 9 days per fortnight.
**What You Need**
We anticipate you will have:
- Bachelor’s degree in Social Work or Nursing, or qualifications for Community Development Worker.
- Experience in working with clients with complex needs. This may include community-based services, such as the housing sector, AOD, disability or Mental Health.
- An applied understanding of the issues and implications of homelessness for clients with complex health and social needs.
- Excellent IT, computer systems and Microsoft Office skills and the ability to competently learn and process new information and systems.
- Valid driver’s license and ability to transport clients where required (fleet cars available).
