**What You Will Be Doing**
The position supports Aboriginal and Torres Strait Islander people, across the local government areas of Frankston City Council and the Mornington Peninsula Shire, with complex care issues by managing, supporting and linking them into appropriate services and programs to improve social and emotional wellbeing and improve health outcomes. The role has a strong client advocacy focus and a primary function of assisting clients to navigate and access the health, housing and the welfare system
This position works within the Mobile Integrated (MI) Health Team, which has a focus on working with people who are homeless, those living in low cost housing and insecure housing.
Key Responsiblities
- The Aboriginal and Torres Strait Islander Access and Support Worker will coordinate the successful engagement of Aboriginal and Torres Strait Islander (ATSI) people into Peninsula Health programs and external services as required.
- Engage a broad range Aboriginal and Torres Strait Islander people via assertive outreach across the community and in settings such as the gathering places.
- Engage and work with Aboriginal and Torres Strait Islander staff and stakeholders from other programs to ensure optimal outcomes for clients.
**What You Need**
We anticipate you will have:
- Identify as Aboriginal and/or Torres Strait Islander background.
- Demonstrated understanding and commitment to Victorian Aboriginal and Torres Strait Islander health and culture.
- Relevant tertiary qualifications in Social Work, Community Development or Nursing (RN).
- Communicates well with, relates to and sees issues from the perspective of, people from a diverse range of cultures and backgrounds.
- Experience working with and understanding of inter-generational trauma and the impact of this on the health and social issues affecting Aboriginal and Torres Strait Islander people.
- Substantial experience in working with complex clients in community-based services, in the homelessness sector or mental health, disability or drug and alcohol areas.
- Highly developed verbal and written communication skills.
- Experience and ability to work with a wide range of services.
- Competent with a range of computer and data entry software.
- Current Victorian drivers licence and willingness to transport clients as required.
