- Opportunity to work across youth, adult and aged community teams
- 2 x Permanent Full Time Positions - 80 hours per fortnight (negotiable)
- Develop comprehensive recovery and treatment plans with consumers

**About the service**

Barwon Health’s Adult Continuing Care Services (CCS) provides community supports to people aged between 16 and 65 years, experiencing serious mental health needs. Our service delivery includes comprehensive crisis assessment, crisis management and treatment, case management of complex issues and mobile outreach. We have community case teams situated across the Barwon region and you will have the opportunity to provide cover across all of the below listed sites:

- South Community Mental Health Team (Bellarine & Torquay)
- North Community Mental Health Team (Corio & Geelong West)
- Aged Psychiatry Service
- Jigsaw and Colac Mental Health Service

**About you**
- Current Australian Driver’s Licence
- A demonstrated high level of clinical skills in the assessment, treatment and continuing care of adults with mental health disorders in a community setting
- Thorough knowledge of the objectives and provisions of the Mental Health Act (2014) and amendments

**About the roles**

The clinician’s role includes;
- Development of a comprehensive recovery and treatment plan with the consumer
- Outreach activities
- Consumer and carer education, support and follow-up
- Assessment of consumers' medical, psychosocial and current service needs
- Development, implementation and evaluation of Treatment & Care Plans
- Advocacy
- Referrals and assistance with linkage to medical, NDIA, mental health, substance use services and psychosocial services as needed
- Please note in these roles, successful applicants will report to the Continuing Case Services Program Manager, who will allocate holiday relief._
