**About the facility**

Barwon Health’s Adult Continuing Care Services (CCS) provides community supports to people aged between 16 and 65 years, experiencing serious mental health needs.

Our service delivery includes comprehensive crisis assessment, crisis management and treatment, case management of complex issues and mobile outreach. We have community case teams situated in five locations across the Barwon region ranging from Corio and Geelong West to Bellarine, Torquay & Colac. Each team is situated in a community health centre that is close to the city centres, beaches or local community hubs.

**About you**
- Formal professionally recognised qualifications in Nursing, Occupational Therapy, Social Work or Clinical Psychology, and APRHA registration where required.
- Current registration as a Registered Nurse and hold a post graduate diploma in psychiatric/mental health nursing, or a specialist course of psychiatric nurse training leading to previous registration as an RPN e.g. Bachelor of Psychiatric Nursing
- Able to work a 7 day rotating roster that consists of weekend work, day shifts (0830-1700) and evening shifts (1130-2000 and 1030-1900) or (1200-2000 and 1100-1900) for Allied heath staff employed under the Victorian Public Mental Health Services Enterprise Agreement.
- Excellent verbal and written communication skills

**About the role**

To provide assessment, treatment and recovery based interventions to individuals and their families who experience serious mental illness. The broader service delivery model of MHDAS includes provision of high quality interventions that are consumer-centred, family/carer inclusive and are delivered in a timely and coordinated manner in line with the Victorian Mental Health and Wellbeing Act 2022.
