**About the Role**:
The Transition Pathways Officer has a unique skillset combining understanding of the departmental policies combined with experience in supporting young people, their families and schools. They possess a thorough knowledge of the services available through Education Queensland and the local community to assist students in their transition from school to an eligible post-school education, training, or employment option.
Involving a Transition Pathways Officer provides the level of support that a young person who has disengaged from school needs to provide assistance at this critical time. A suitable pathway will be determined based on that student's particular requirements which could result in a return to traditional education or a suitable post-school education, training, or employment option.
Transition Pathways Officers submit monthly case management reports to the Deputy Principal - Regional Engagement which are collated for each school and cluster and reported back to ARDs, Director and Regional Director.
Transition Pathways Officers work with their schools to ensure that students who transition from schooling prior to the end of year 12 are able to walk out the front door of the school with dignity.
As the Transition Pathways Officer, you will have responsibility for the following:

- Support schools to interpret, develop, refine, and enact departmental policies, practices and procedures relating to students exiting school prior to the completion of Year 12 to ensure they remain in an eligible option.
- Provide support to schools that will assist in the development and implementation of successful retention and transition strategies and programs that will improve the transition outcomes for 'at risk' students.
- Provide appropriate support through the identification of student needs options and intervention to young people and their families to assist in the transition of young people into a viable education, training, or employment.
- Collate and analyse confidential information, identifying needs and barriers to formulate the basis for the development of a school to post school transition plan for young people (post compulsory participation phase) who leave school before the completion of Year 12 (transition of students in the compulsory participation phase to be determined by region) and provide appropriate case management, advocacy and support as required.
- Forge links with providers of employment services, community agencies, training organisations and employers to create opportunities for young people to engage in programs that support successful transitions from school into employment or training.
