About the role
Working closely with the Home Manager, the Clinical Care Manager will lead the Clinical team, support the Home Manager to create and maintain an environment where residents feel valued, treated with dignity and respect and their individual needs, wishes and goals are met. This is a Full-Time Position working in Sunbury, supporting our residents to live their best life.
Key Responsibilities
Oversees and ensures compliance of the development and ongoing assessment of resident care plans
Ensure management of care for residents including those with complex needs such as pain, wound and behaviour management
Ensure systems and processes are developed and maintained to ensure the quality and safety care, optimal outcomes and excellent resident experience across the home.
Ensure system in place to identify deterioration and changes to care needs and organise reassessment and care plan changes
Maintain a relationship with medical practitioners, Pharmacy and allied health to ensure appropriate care and services to meet the residents needs
About you
Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
An effective listener with the ability to communicate at all levels both orally and in writing with excellent presentation skills.
Understanding of and ability to manage human resource issues with effective negotiation skills.
Ability to understand and use information technology and programs and to learn new skills as required.
An inherit curiosity and a personal value of wanting to do better all the time, a change leader.
Working knowledge of WH&S and Infection Control
A minimum of 3 years’ experience in a senior management role within the aged care sector with a proven track record
Quality Assurance Certificate or equivalent experience
A proven ability in quality management using a teambuilding approach through mentoring and staff development
A high level of commitment, integrity, honesty and trustworthiness.
