**This is a temporary full time 38 hours per week position until 15th June 2025 with the possibility of an extension.**


**What you'll be doing**

The Transitional Aged Care Program Manager is responsible and accountable for the effective day to day management of the Transitional Aged Care Program (TACP). TACP is a Commonwealth and State Government program, provided to clients in the community by the Illawarra Shoalhaven Local Health District. The role of the Manager is to provide expert leadership to the Nursing, Allied Health and Administration staff employed within the service.
The program provides therapy such as physiotherapy, occupational therapy, social work, dietetics, speech, and psychology as well as nursing and care/domestic services The program is time limited (up to 12 weeks), reablement focused and targeted towards older people who may otherwise enter residential care early. There are 63 places available, numbers can fluctuate according to MoH allocations for community places within the Illawarra LGA.
The position holder will need to demonstrate sound knowledge of the Aged Act and Aged Care Principles and current knowledge of commonwealth funded services. The position involves oversight of all referrals, ensuring co-ordination of all allocations and monitoring the care status of clients in the program, geriatrician case conferencing as required, and working closely with multiple referring hospitals. The role has financial responsibilities including monthly reporting to Ministry of Health, HR and Clinical Governance responsibilities and strong liaison and negotiation with Hospital Access and Flow managers, Hospital Staff, Aged Care Assessment team and Aged Care Service providers.

Selection Criteria
- Current National Registration in Allied Health or Nursing (Division 1) with the Australian Health Practitioners Regulation Agency (AHPRA).
- Proven recent experience working in Clinical/Aged Care/ Community care service provision.
- Relevant post graduate qualifications or working towards the same in Management or relevant Management experience.
- Demonstrated sound knowledge of the Aged Act and Aged Care Principles and current knowledge of commonwealth funded services. Being aware of the Guidelines and legislation that covers this program.
- Demonstrated ability to evaluate clinical services and manage change.
- Proven ability to create and maintain a positive workplace culture and the ability to lead a multidisciplinary team in achieving organisational goals.
- Demonstrated computer literacy and confidence in using Microsoft and health care information systems.
- Current Class C drivers licence.
