About the Role
The Targeted Care Case Manager (Key Worker) provides support and intervention to parents and carers for children and young people who are at risk of entering into care or who are transitioning from care. The Case Manager supports and conducts works in partnership with Child Protection and other support services, as well as children, young people, parents, and carers to provide a holistic trauma informed service using the Best Interests Case Practice Framework.
A key focus of the role is to partner with support services to provide;
case management of children and young people on Family Options packages,
conducting family reunification and preservation assessments,
carers reviews, case plan development and review,
monitoring of care and providing case management for contracted Child Protection clients where appropriate
The Key Worker will be a part of Baptcare’s Community Services Victoria segment, which offers a range of case management, assessment, brokerage support, planning and respite activities to families.
About You
You are passionate about continuing to grow your career in the Family and Community Services space coming from a leadership role. Solutions focused and up for making a difference, you align your approach to strategy to achieve organisational and divisional goals.
**You will ideally bring the following**:
Tertiary Qualifications in Social Work, Psychology, Counselling, or other relevant Human Services qualification and/or experience
Experience in providing therapeutic interventions
Skills and experience relevant to Recommendation 209 requirements
Key Selection Criteria
Demonstrated case management skills with complex families and at-risk children and young people
Capacity to work effectively with families from a range of cultural and socio-economic backgrounds
Knowledge of the legislative and political context, with ability to research and seek information from stakeholders to support and inform assessment of risk and service efficacy
Demonstrated ability to identify, assess and prioritise risks and needs of clients in a holistic approach to practice, to adapt to meet a changing environment and needs of carer and/or client group while maintaining focus on service delivery
Strong written and verbal communication skills, including negotiation and problem-solving
Ability to work within a multi-disciplinary team environment by sharing information, supporting, and respecting colleagues
Sound organisational and time management skills
Working from a strength-based perspective
