**About the role**

The Mental Health Directorate works with youth, adults and older people and provides a stepped range of services encompassing emergency, acute inpatient, sub-acute, community and rehabilitation services.

The position is located in the Western Community Mental WCMHS) in the Assessment and Brief Intervention Service (ABIS) based at Woodville. The position works across 7 days, including out of hours, weekends and public holidays. As part of a stepped service, the team focusses a significant part of its service delivery for consumers in the Acute Phase of Care.

Working as a part of the integrated multi-disciplinary team of community mental health service, the Social Worker undertakes care coordination and is responsible for the delivery of a range of other clinical activities including care planning, clinical review, psychosocial assessment and mental health assessments consistent with the Clinical Business Rules.

The Social Worker works collaboratively with consumers, families/carers with the aim of maximising their wellbeing, assisting them to stabilise their mental health and building their capacity for self-management. The aim is to enable the consumer to participate in a range of valued community activities and facilitate improved functioning and community tenure.

The Social Worker provides specialist assessment, therapeutic and family/carer services, crisis management and advocacy. The Social Worker liaises closely with key stakeholders, including other mental health teams, primary care providers and other government and non-government organisations that support consumers with mental disorders.

This role may also provide opportunities for the Social Worker to participate in service improvement, evaluation of clinical services and undertake appropriate education and research activities.

**Qualifications**
- Appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.

**Essential Requirements**:
