- **Temporary Full-Time position working up to 31 March 2023 (with possibility of extension).**

You will provide a holistic support service to Aboriginal clients in the Gawler community. Working in a collaborative and participative framework to increase continuity of care across the health setting, you will strive to address social issues and promote better outcomes for Aboriginal people through the facilitation of Well Health Checks. You will play a vital role in networking and maintaining collaborative partnerships between service providers, non-government agencies and the community throughout the Gawler service area. Contributing to service development activities and evaluating service effectiveness, you will assist with monitoring care plans to ensure appropriate and relevant care outcomes are achieved on a daily basis. Promoting positive health in the community, we are developing culturally appropriate models of care for Aboriginal Health and extending Aboriginal community engagement. Increasing access and equity to health services, we are committed to addressing the social determinants of Aboriginal health.

You will hold a Certificate III in Aboriginal and or Torres Strait Islander Primary Health Care (Practice or Community Care), or be working towards this qualification. You will have demonstrated experience working in a health-related field with Aboriginal people and communities. This will be supported by your ability to work collaboratively and contribute to a partnership approach to program activities. Sound communication and interpersonal skills will be essential, allowing you to interact effectively with management, staff, customers, external organisations and community groups. Basic administration skills including computing, data entry, filing and case note compilation will also hold you in good stead for this role.
