**The opportunity**
Vacenti is seeking a full-time Care Coordinator to join the clinical team at our Portofino Facility located at Hamilton, QLD. Reporting to, and working closely with the Clinical Manger, this role will supervise the day-to-day delivery of care and services to our residents, whilst providing clinical guidance and support to care and allied health staff. The Care Coordinator ensures the delivery of person-centred care that meets the individual clinical, social, and emotional needs of our residents.
**Your main responsibilities will include**:
- Providing clinical expertise and decision making to promote the best possible nursing practice to residents.
- Providing leadership, coaching, mentoring and supervision to registered nurses.
- Actively involved in the delivery of care and services to residents.
- Actively participating in the quality management systems of the home including data analysis and trends.
- Promoting and coordinating clinical documentation including, assessment, care planning and evaluation and all aspects of AN-ACC assessments and iCare documentation.
- Checking progress notes and other documentations daily.
- Review and manage incidents when/if they occur.
- Support residents, families, and teams in coordinating case conferencing
**To be successful in this position you will need**:
- Relevant experience in a senior nursing position.
- Ideally you will need in-depth understanding of AN-ACC and quality assurance requirements that operate in aged care and sound supervision, organisational and communication skills.
- Current registration with AHPRA is required.
- To be considered for this opportunity you must have the right to live and work in Australia. If you are successful, you will be required to obtain a national police clearance prior to commencement.
