About the role

We are seeking a dedicated Case Worker for The Doorways Program. This position will support community members to meet their goals through community partnerships, collaboration and using case management as a tool.

Key Responsibilities
- Assess community members’ specific needs of interrelated issues and needs
- Develop high quality case plans with community members to identify achievable goals
- Use early intervention strategy to deliver support to people experiencing disadvantage
- Provide effective response to people experiencing longer term disadvantage
- Support community members to build and increase capacity and develop independent living skills towards self determination

This is a permanent part-time position, (14 hours per week), based in Ballarat, VIC. Salary and conditions are in accordance with SCHADS Level 4.

What you will bring
- Degree level qualification in Social Work, Psychology, or other Community/Human Services discipline is highly desirable, or a Diploma and a minimum of 2 years relevant experience in the community services sector would be considered
- Supports others in challenging situations and acknowledges outcomes that were achieved by effective collaboration
- Remains open to new ideas and approaches
- National Criminal History Check
- Working with Children’s Check
