The role you could play
We are seeking a dedicated Case Manager for our Housing Connect program The Salvation Army Supported Housing (SASH) program is part of the Homelessness Stream which supports those who are homeless. As the Case Manager you will be responsible to provide specialist case management to those in housing need and provide support between the tenant and the landlord.
Duties
- Provide flexible and individual client driven case management in areas of need, identified through initial and on-going assessments
- Access translation and interpretation services as required for clients
- Individual case plans are independently developed in collaboration with clients and include a range of interventions
- Progress of individual case plans are regularly monitored
- Build productive working relationships with internal and external stakeholders and work with multiple services across a range of forums
- Develop strong industry links with the community and housing sectors in Tasmania
- Contribute to build a strong culture in the wider homelessness stream in Tasmania, based on mutual respect and collaboration
This is a max-term full-time position, 12 months from start date, based in New Town, TAS. Salary and conditions are in accordance with SCHADS Level 5 Award.
- Degree qualification in Social Work, Welfare, Community Services or related field OR demonstrated high level knowledge of the Housing and Homelessness sector in Tasmania through previous appointments, service and study/development
- Demonstrated experience in working within a psychosocial framework with homeless clients who may have complex mental health, health and or addiction issues, and intellectual and physical disabilities
- Highly developed Case Management, Case Work and Case Planning skills
- High-level oral and written communication skills
- Knowledge of models of homelessness intervention
- Ability to collaborate with people who have complex needs and challenging behaviour
- Current Tasmanian Driver's Licence
- Current First Aid Certificate
