**Description**:
An opportunity has arisen at Right at Home Sydney Northern Beaches for an energetic self starter, who is a highly motivated and experienced individual, to join our business as a **Clinical Care Coordinator (CCC)**on a Full Time basis.

We are seeking to recruit dedicated individuals who strive for best practice, are able to communicate well with people of all backgrounds and approach every situation with care and compassion. If you're a self-motivated person with a positive attitude who enjoys and thrives on challenges, we would love to hear from you.

The Care Coordinator will be responsible for:

- Building strong connections with clients and their families
- Assessing client needs and resources for health, welfare and other facilities and services
- Conducting Client Care Assessments and Care Planning
- Maintaining accurate records as per the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework
- Supporting all clients to meet their goals
- Supporting and providing feedback to Home Care Workers
- Re-organising work priorities to meet the needs of a fast-paced environment
- Must promote the values of the company and prepared to plan supports using a contemporary approach

**Qualifications**:

- A current registration certificate for a Registered Nurse.
- Excellent skills in assessing, documenting and implementing client's care planning objectives.
- Experienced in assessing and planning care needs
- Sound understanding of the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework
- Organisational, administrative and time management skills
- Ability to work in a team setting, both as a participant and a leader.
- Well developed problem solving, interpersonal, written and communication skills.
- COVID 19 Vaccinations

Ability to Obtain:

- National Police Check
- Drivers Licence
- NDIS Worker Screening Check
- First Aid & CPR Certificate

Licence/Certification:

- National Police Check (preferred)
- COVID Vaccination (preferred)

Work Authorisation:

- Australia (preferred)

**Preferred Skills**:

- AHPRA Registered Nurse registration with previous leadership experience
- Staff mentorship & development experience.
- Up to date working knowledge & experience achieving accreditation standards
- Ability to identify & improve areas of clinical risk & quality guidelines
- Consultative & effective communication skills.
