**About us**:
Premium Disability Support Services is an NDIS-registered Disability Services provider operating in Victoria. We value **commitment, punctuality, ethics and honesty** in an individual. We are seeking an exceptional person to provide support and assistance to our clients.

We are currently looking for a full-time **Support Coordinator** to join our growing team.

**Your role**
- Manage and implement the participant’s plan.
- Develop roster of care based on client's goals and funding availability.
- Achieve the outcomes identified in the participant’s plan.
- Connect the participant with formal, mainstream, and funded supports.
- Bring in more business to the organisation via your industry connections/contacts and goodwill.

**About You**:

- Minimum Certificate IV in Disability, Mental Health or other qualifications relevant to the field of human services with relevant experience.
- Proven experience working with people with disability and/or mental illness.
- Demonstrated ability to network with local service providers.
- Demonstrated experience in support planning, working within a person-centred framework.
- Well-developed interpersonal and communication skills, both written and verbal
- Sound organisational and administrative ability

**To be considered for the role applicants must have the following (or be able to obtain upon request)**:

- A minimum of 12 months of experience providing NDIS support coordination and supporting people with physical, intellectual, or psycho-social disabilities.
- Qualification in Mental Health, Community Services, Disability Services, or related industry OR experience in same.
- Thorough understanding of SIL and SDA.
- NDIS procedures such as Accessing the scheme, conducting planning meetings and reviews, report writing.
- Strong willingness to bring in more participants and maintain the existing clientele.
- Great network of Allied Health Professionals.
- Guiding people to develop goals and aspirations for their lives, and to achieve their ambitions
- Researching, and linking people with, funded supports along with informal, mainstream and community opportunities
- Strengthening customer's capacity to develop relationships, live more independently and be part of their community
- Negotiating with, and monitoring quality of, providers
- Assisting families through periods of crisis
- Evaluating services and funding and continually seeking growth opportunities for clients.

**Other requirements**:

- Rights to work in Australia.
- Completion of the NDIS Quality and Safeguards online worker training module.
- Have a decent car (preferred).

**Job Types**: Full-time, Part-time, Permanent, Fixed term, Subcontract, Temp to perm, Contract
Contract length: 12 months

Pay: $65,000.00 - $75,000.00 per year

Expected hours: No less than 35 per week
