**About the Role**
As part of this role you would be responsible for:

- Assessing clients for eligibility for the HACC Program for Younger People, including the HACC Flexible Response Program
- Using a wellness and reablement approach, undertake and document in-home assessments, develop support plans, complete care plan reviews and provide support for younger people and their carers
- Liaise with and refer to internal and external services, maintaining up to date knowledge of services and supports for younger people and their carers.

**Skills & Experience**
- A tertiary degree qualification in human services, nursing, allied health or related discipline.
- Comprehensive experience in a community assessment and/or and community care role
- Sound knowledge of and skills in developing creative and goal directed support plans
- Demonstrated ability to provide support for vulnerable people experiencing issues such as homelessness, mental health issues, drug and alcohol issues, elder and systems abuse, neglect, financial disadvantage and cognitive impairment
- Demonstrated capacity and commitment to actively contribute to continuous quality improvement
- Excellent Communication Skills with demonstrated ability to engage and communicate with a range of people and for a range of purposes, verbally and in writing.
- Current Victorian driver’s licence
