**Role**: Support and Outreach Coordinator - Clifton, 4361

**Hours**: 4-5 hours per week - Part Time
Must be available Monday to Friday

**Key Responsibilities**:
**Support Responsibilities**:

- Provide in-home support to individuals with disabilities, including assistance with daily routines and personal care.
- Deliver community access support to help clients engage in social and community activities.
- Support client well-being by helping manage behaviors of concern and ensuring a safe and positive environment.
- Collaborate with the participant’s family and National Disability Care team to implement individualized support strategies tailored to the client’s needs.
- Assist with home cleaning tasks as required, ensuring a comfortable and clean living space.

**Outreach Responsibilities**:

- Promote National Disability Care services within the Clifton community through localized outreach and business-to-business connections.
- Use social media platforms to increase awareness and engagement of services.
- Combine support work with outreach activities to build stronger community ties and expand service reach.
- Collaborate with a growing and innovative team to help raise awareness about National Disability Care.

**Requirements**:

- Outgoing, positive, and resourceful attitude.
- Self-motivated, with the ability to work independently and as part of a team.
- Experience working as an NDIS Support Worker, with a focus on both support and outreach activities.
- Female preferred (client's preference).

**Skills and Qualifications**:

- Must meet the client’s preference (female preferred for this role).
- 100 Points of Identification.
- Valid Driver's License with access to a reliable vehicle.
- Car Insurance and Registration.
- NDIS Workers Screening Clearance.
- NDIS Workers Orientation Module Certificate.
- Police Check.
- Working With Children Check (WWCC).
- First Aid and CPR Certification.
- Relevant experience in the disability sector, with a focus on support work and outreach.

**Job Type**: Part-time

Pay: $38.08 per hour

Expected hours: 4 - 5 per week
