A little about the role
We have a total of 1.0 EFT hours for 12 months available for a Diabetes Nurse Educator with experience in the community health sector.
Join our dynamic, multi-disciplinary team to provide diabetes assessment, education, and self
- management support services to our diverse community.
- Your day-to-day responsibilities will include:
- Providing evidence based, client centred, goal directed assessment and care planning to a diverse caseload of clients
- Contributing to the multi-disciplinary team, which may include team-based care plans, case reviews and initiating referrals as appropriate
- Meeting organisational and funding KPIs
- Communicating effectively with clients, colleagues, and other stakeholders
**Desired Skills and Experience**:
About you
You will have experience working as a Diabetes Educator within a community setting. You will also have:
- Post Graduate Certificate in Diabetes Education.
- ADEA Credentialed Diabetes Educator (CDE) status or working towards credentialing.
- Registered Nurse Division 1 holding current registration with the Australian Health Practitioners Regulation Agency, with a minimum of 1 year’s post qualification clinical experience.
- Well-developed clinical skills and demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes as agreed with client,
- Experience in planning, implementing, and evaluating innovative, evidence based, clinical services
- Experience working with clients from diverse cultural and social backgrounds and working with interpreters.
