As a Disability Team Leader, you will play a crucial role in providing support, advocacy, and care to individuals with disabilities, enabling them to lead fulfilling and independent lives. You will work with the team to strengthen all life areas of individuals, families, and communities through developing and maintaining efficient and effective services. This involves planning and evaluation of operations including providing analysis of work practices and the individual outcomes.

Your responsibilities will include:

- Support Individuals to participate in age-appropriate activities and tasks including employment, in home support and community access.
- Support Individuals to access activities and outings in the community.
- Provide direct care assistance to each Individual in accordance with their service plan and as directed by the Manager.
- Work in accordance with the medication administration policy, work health and safety procedures and Individual rights policies including privacy and confidentiality.
- Monitor the safety and well-being of the Individuals and report any concerns to the Manager.
- Provide 1:1 or small group training and support in response to individual needs.
- Ensure completion of appropriate documentation.
- Participate in the Goals Planning process for individuals and monitor outcomes.
- Work with Individuals, family/ carers, relevant case managers and other professionals to support the Individual’s individual goals.
- Ensure that all programming undertaken is appropriately maintained, documented, and reviewed.
- Preparing a monthly report on Individual progress.
- Contribute to Case reviews with Employment Consultants and Facilitators and allied health teams as required.’
- Maintain positive welcoming relationships with family, friends and other service Providers of Mirrors Care customers.

Essential Criteria:

- Minimum of certificate IV or Diploma qualification in Disability Services, Community Services, Mental Health, or a related field or staying towards
- At least 12 months of experience in a similar role.
- Competency using computers and basic Microsoft programs.
- Working with children check
- NDIS Worker Screening Check
- Full drivers' license

- Demonstrates a high level of commitment and responsibility whilst understanding and respecting Mirrors Care customers, their families and other related people.
- Works effectively in a team and within a leadership role, communicates well and shows continued enthusiasm for developing Mirrors Care services through effective customer service.
- Dedicated support, training, and communication skills to provide support, development, and assistance to colleagues.
- Well-developed conflict resolution skills
- Identify problems; seek to understand their underlying causes with the ability to act quickly and decisively to resolve them.

**Salary**: $70,000.00 - $85,000.00 per year
