The **Lifestyle Co-ordinator** is responsible for assessing, planning and facilitating lifestyle engagement and practice at our Aged Care Home. This role will see you developing and coordinating the delivery of our Lifestyle Programs through a dedicated team and to work closely with on site physiotherapists in the delivery of wellness programs.

**Join us as a member of the leadership team in our Sylvania home**. As the Lifestyle Coordinator you will work closely with the Care Manager, and leadership team to provide a high standard of lifestyle and wellness services to residents.

**About the role**:

- Broad and varied autonomous role
- Develop lifestyle calendar and programs
- Develop lifestyle care plans suited to residents’ abilities and preferences
- Member of the facility Leadership Team

**About you**:

- Cert IV in Leisure and Health
- Experience in a similar role
- Good communication skills, listening, written and oral
- Genuine interest in talking to people and ability to encourage others to enjoy their lives to their full capacity
- Ability to work as part of a team
- Excellent organisational and time management skills
- Must have valid working visa
- Current Police Check (or willingness to obtain)
- Current Influenza and COVID-19 vaccination
