**The Department**

The Caulfield Community Health Service (CCHS) provides a range of clinical and health promotion services for people of all ages who live, work or study within the local community. These multi-disciplinary services are provided at home, in the community or centre, or via telehealth. CCHS operates from the Caulfield Hospital site. Health Promotion is a key component of the services provided by CCHS.

**The Role**

As Health Promotion Coordinator (HPC) you will be responsible for leading and coordinating health promotion services. You will be required to demonstrate and develop best practices while integrating health promotion across CCHS. You will work closely with stakeholders to implement and evaluate various CCHS health promotion strategies across the community.

You will be familiar with contemporary health promotion theory and practice, including complex systems and settings based approaches to lead and drive a number of health promotion projects and activities across a range of priority settings within CCHS.

You will have the opportunity to work closely with the Population Health Team. You will be responsible for ensuring strategic planning, coordination, implementation and evaluation of action plans, projects and initiatives. All of which will aim to build and empower individuals in order to improve their health and wellbeing for all.

**Skills/ Qualifications Required**
- Ability to develop and support partnerships and to work with a range of stakeholders
- Ability to actively representing CCHS on a range of internal and external networks.
- Ability to provide professional supervision and support to Health Promotion Officers, short
- term project staff, students and volunteers.
- Demonstrate a strong development focus.
