**About the Program**

Under the Primary Health Networks and as part of Connecting Senior Australians to Aged Care Services the Care Finder Program targets, support and assists senior Australians to navigate and access the aged care system, relevant community supports and providers.

**About the Role and You**

As Community Connections Program Coordinator you will practice assertive outreach, explore and establish different ways to engage with people in the Care Finder target populations and connect them to services within the community. Supporting senior Australians who need intensive support who could otherwise fall through the cracks to engage with aged care services and other relevant community services. You will be an engaging and collaborative networker with a passion for ensuring that the services required within this sector are available to the CALD and Australian Aged communities.
- Experienced in assertive out reach in the Logan LGA to actively engage with specialist and intensive assistance services, who can provide their services in a culturally appropriate way, for the aged population from both CALD backgrounds and Australians.
- Identifying and engaging with clients from the aged care sector, especially Cultural and Linguistic Diverse persons and link them to the specialist services.
- Providing specialist and intensive assistance to support aged clients to understand and access My Aged Care assessment, aged care services and connect with relevant services in the community.
- Monitor processes in the sector and ensure that the services provided are assessed and appropriate and changing if required
- Experience of building, maintaining and leveraging established networks and connections in the local community to support the identification and engagement of potential clients and the community supporters and providers
- You will have relevant qualifications in social work, health, age care and or human services.
- Have previous experience in supporting clients to understand and complete community services and provider forms
- A background in case management of clients with a strong emphasis on record keeping
- Excellent communication and inter personal skills are required with the ability to negotiate, advocate and problem solve
- A good track record of supporting and engaging in continuous improvement practices
