**Berry Streets Child, Youth and Family Services Team has partnered with Latrobe Community Health Service (LCHS) to establish a new coordinated health and welfare service for vulnerable children and families in Latrobe City.**:
**Together with the LCHS Care Coordinator, the Social Services Navigator will collaborate with other Integrated Health Care Hub professionals to support families and address barriers to engagement with multidisciplinary health services.**:
**Core responsibilities**:

- Empowering disadvantaged children and their families, who present with varying vulnerability factors, including having a broad understanding of issues impacting children's health, safety and development.
- Establishment, implementation, and coordination of the program.
- System navigation support for service users and professionals.
- Workforce planning and providing continuous quality improvement and program evaluation.
- Providing leadership and coordination in social services intake, assessment, care planning and referral.

**This full-time fixed-term position offers until 30 June 2025, offering a base annual salary of $104K plus super and the option of salary packaging to increase your take home pay.**

**This role will be co-located at Latrobe Community Health Service, Churchill and Berry Street’s Morwell office in the South-East Victoria region on Gunai Kurnai Country.**
