**Family Preservation and Reunification Worker**
Full Time - 38 Hours per week until 30/06/2024- Colac Area Health is a values based organisation providing a unique integrated health service comprising of over 500 employees, providing Acute Care, Aged Care, Community and Allied Health Services.- The Family Preservation and Reunification Response (FPRR) Program will provide a rapid, intensive and sustained intervention aimed at preventing at risk children entering or re - entering the care system. This response will link to, and build on existing child and family services within the broader service system by providing an enhanced continuum of care and additional support whilst achieving measurable outcomes. The role is critical in implementing a wrap around, evidence informed case management approach.**What’s required?**
- Minimum of two years’ experience in case management.
- Comprehensive understanding of Child and Family Services, Child Protection, relevant legislation and court orders.
- Experience in delivering strengths based risk and needs assessment with families and managing associated risk. Strong skills in building relationships, partnerships and networking to further agency and program objectives and improve client outcomes.
- Excellent communicator with strong interpersonal skills and advanced written and oral communication.
- Self-starter with a solution focussed approach.
- Proficient use of MS Office, IRIS, and knowledge of a range of IT platforms.
- Ability to identify opportunities for continuous improvement and implement processes and systems to improve quality of service.
- Current full Victorian Drivers Licence, satisfactory criminal records check and WWCC.
