**Description**:
Purpose
Providing individualised care and support to adults and young people with a disability, ensuring the wellbeing and the safety of the adult or the young person is paramount while complying with the Disability Services Act and registration standards within New South Wales.

Primary Responsibilities
- Provide personalised support/care to Adults/young people within the community.
- Work as part of the broader person centred team, including teach/induct new workers to the support provided for the Adult/young person.
- Liaise with the Disability Coordinator regarding any support/care related issues, including advising the relevant Coordinator/AH Coordinator immediately when the worker becomes aware they may not be able to attend a shift.
- Complete relevant paperwork in regards to the Adult/Young Person - this must be done prior to the completion of each shift.
- Administration duties, such as managing finances, receipts and invoices received during shift, maintaining client files in the house, use of up-to-date forms.
- Application of the Person Centred/positive Behaviour Support plan in all day-to-day tasks.
- Providing guidance to the Adult/young person around positive life choices.
- General household duties, including but not limited to cleaning, shopping and general maintenance.
- Attend training sessions.
- Attend House Meetings meeting’s.
- Be conversant with the ITEC Health Aged and Disability Manual and comply with all ITEC Health Policies & Procedures.

Financial Management
- As an employee of ITEC Health, the Support Worker has a responsibility to ensure that a clients’ personal spending is recorded and administered in a way that is accurate and must provide receipts for all purchases.
- Accountability
- Accept responsibility & accountability for the operational tasks associated with this role at all times.
- Ensure ownership of the actions, outcomes and strategy of ITEC Health through transparency and virtuous activities.
- Maintain confidentiality and adherence to all organisational policies and procedures.

Quality & Risk Management
- Ensure all tasks are completed within legislative requirements associated with ITEC Health. Ensure personal compliance with Workplace Health and Safety, Privacy, and relevant Industrial Relations Laws and ITEC Health’s Policies and Procedures.
- Support Innovative Approaches
- The Disability Support Worker is to regularly review practices with a view to continuous improvements at ITEC Health sites, including considerations of environments and processes conducive to the development of the skills and behaviours required for an Adult/young person to live a positive life trajectory.

Reporting

The Disability Support Worker reports to the Disability Coordinator.

Stakeholder Relationships

Via participation in Positive Behaviour Support/Person centred plan meetings and other meetings, the support worker is to maintain appropriate working relationships with a variety of stakeholders, including Disability Services, Public Trustee the Office of the Public Guardian, Natural family members and a range of other service providers in a variety of forums, to ensure that the needs of the Adult and young people in the care of ITEC Health are met.

Workplace Health & Safety
- Comply with the WHSMS and assist in the implementation of WHS and risk management strategies across the organisation.
- Encourage, support and assist with incident reporting, implementation of corrective actions and assist in the management in any injuries.
- Liaise, communicate and consult with directs regarding WHS matters and report findings back to the WHS team.

**Skills and Experiences**:

- Must hold current or be willing to obtain NSW WCC checks, Federal Police Check and any other disability services sector clearance (date issued within 3 years)
- Must hold current or be willing to obtain Provide First Aid Certificate (HLTAID003)
- Formal Qualifications in Certificate III in Individual Support (Disability) or willingness to obtain (highly desirable)
- Must hold current or be willing to obtain Manual Handling Certificate (HLTWHS005, annual)
- Hold current NSW open Drivers Licence and highly desirable to have own insured vehicle
- Knowledge of person centred thinking practice (highly desirable)
- Rights to work in Australia (Citizen or permanent resident)
