**About the role**:
At Uniting AgeWell, Personal Care Workers co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences. Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents’ conditions and escalate observations and concerns.
- Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
- Provide personal care to residents with complex needs such palliative care, dementia support, restorative care and pain management to enhance their wellbeing
- Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
- Monitor the cleanliness, safety, environment and promptly report issues of concern
- Part Time and Casual roles available
**About you**:
This is the place for you if you have:
- Certificate III in Individual Support, or equivalent (Aged Care)
- Experience in residential aged care/ home care (desired)
- Experience with dementia care and palliative care support (desired)
- Demonstrated abilities to support residents in a fast paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners
