Care Team Leader

Care Team Leader - Western Sydney at HammondCare. Full-time opportunity in the Western Sydney HammondCare at Home Team. The role leads the team of Community Care staff who provide direct care to our clients, ensuring staff understand individual client needs and can provide services tailored to the needs of each client, delivered within budget and service expectations. The Care Team Leader is responsible for leading, supporting, and mentoring their team, and works in partnership with the wider team where required.

The role of the Care Team Leader is to lead the team of Community Care staff who provide direct care to our clients. They ensure staff have a good understanding of client needs and preferences and can provide services tailored to individual clients, within budget and service expectations. The role also includes leading, supporting, and mentoring the team, and working in partnership with the wider team where required. The role includes managing the recruitment of new Care Workers and ensuring all working validations, education, development plans and resources are available.

• Lead the team of Community Care staff who provide direct care to clients.
• Ensure staff understand individual client needs and preferences and provide services tailored to each client, within budget and service expectations.
• Lead, support, and mentor the team; collaborate with the wider team as required.
• Manage the recruitment of new Care Workers and ensure validations, education and development plans are in place.

• Certificate IV in Aged Care (Desirable), HR or similar degree
• Minimum of 2 years relevant industry experience
• Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme
• Performance coaching experience and management of a team while operating in a multi-site environment
• A current driver's licence and safe driving record and willingness to travel between HammondCare's clients, sites, and services
• Strong organisational, administrative and computer skills
• Empathy and patience
