We are seeking a Disability Support Worker in Frankston, Victoria for Social and Community activities.

Our customer is in his mid-50s, is hearing impaired, and lives with chronic pain. He and his family are looking for a compassionate and confident support worker, who can plan ahead of time and develop capacity-building and goal-orientated community access activities.

The shifts will be school hours Monday to Friday and occasional weekends.

Minimum Requirements
  1. Proven or lived experience as a Disability Support Worker or in a similar role
  2. Certificate III or IV in Disability Support, a related field, or the ability to obtain
  3. Manual handling training
  4. Knowledge and understanding of disability support principles, policies, and practices
  5. Worker Orientation Module
  6. New Worker NDIS Induction Module
  7. Supporting Effective Communication
  8. Supporting Safe and Enjoyable Meals

Responsibilities
  1. Providing hands-on support and care to individuals with physical and neurological disabilities.
  2. Assisting individuals with daily living activities, including personal hygiene, meal preparation, medication administration, and household tasks.
  3. Promoting independence and facilitating community participation.
  4. Following personalised care plans to meet the unique needs of each individual.
  5. Collaborating with multidisciplinary teams to ensure holistic care and support.
  6. Maintain accurate and timely documentation of care provided and report any changes in individual needs.
  7. Adhering to all safety protocols and guidelines to guarantee a safe working environment for yourself and the people we support.

You must have or have the ability to obtain:

  1. First Aid and CPR certification.
  2. NDIS Worker Screening Check
  3. Police Check
  4. Working with Children's Check

You must have:

  1. The ability to communicate effectively with individuals with disability, their families, and other stakeholders such as OT's, Physio's etc.
  2. Strong interpersonal skills and the ability to build positive and professional relationships with the people we support.
  3. The ability to work independently, as well as part of a team.
  4. Excellent problem-solving skills and the ability to think on your feet in challenging situations.
  5. Flexibility to work various shifts including weekends, evenings, and holidays.
