OverviewYou will be working as a clinician in APATT, the community aged mental health team, and will be responsible for psychiatric assessment and treatment.
You will visit clients at home, provide intake and triage, crisis support, assessment and case management, review risk and provide support to reduce impact of illness and enhance independence for clients and carers in a variety of community and residential settings. You will provide discipline-specific support and advice to the team, using a cooperative and recovery-focused approach across the continuum of care to achieve optimal outcomes for consumers.
Responsibilities- Visit clients at home and provide intake and triage, crisis support, assessment and case management.
- Review and manage risk; provide support to reduce the impact of illness and enhance independence for clients and carers in community and residential settings.
- Provide discipline-specific support and advice to the team; follow a cooperative, recovery-focused approach across the continuum of care.
- Relevant degree and/or postgraduate qualifications
- Relevant AHPRA registration
- 2 years post-qualification clinical experience within Mental Health and/or aged care clinical services including case management and care coordination experience
- Demonstrated passion for achieving the best clinical practice
- Knowledge of Mental Health and Wellbeing Act Victoria and other relevant legislation
- A valid Nationally Coordinated Criminal History Check, or willing to obtain
- A focus on wellbeing initiatives, with regular events and programs
