Care Team Manager Role Overview

As a Care Team Manager with HammondCare, you will provide professional leadership and support to your team in delivering high-quality care services to our clients. This is a full-time opportunity in the Nepean/Blue Mountains HammondCare at Home Team.

  • Lead a team of Community Care staff who provide direct care to our clients, ensuring staff have a good understanding of individual client needs and preferences.
  • Collaborate with other managers and stakeholders to ensure that services are delivered within budget and service expectations.
  • Recruit, manage, and develop a team of Care Workers, providing them with necessary resources and support.
Key Responsibilities
  • Manage a team of Community Care staff, including recruitment, training, and performance management.
  • Develop and implement plans to meet client needs, working within budget constraints.
  • Collaborate with Clinical Care Manager, Client Liaison Manager, and Regional Area Manager as required.
Requirements
  • Certificate IV in Aged Care or similar degree (Desirable), with a minimum of 2 years relevant industry experience.
  • Knowledge of Home Care Packages Program, CHSP services, Aged Care Quality Standards, and Commonwealth Home Support Programme.
  • Performance coaching experience and ability to manage a team in a multi-site environment.
  • A current driver's licence and willingness to travel between HammondCare sites and services.
  • Strong organisational, administrative, and computer skills.
  • Empathy and patience.
