Are you a passionate Social Worker ready to make a real difference in the lives of young people? At South Western Sydney Local Health District (SWSLHD) Youth Health Services, we're looking for a committed and compassionate professional to join our dynamic multidisciplinary team.

What You'll Do:

  • Deliver client-centred, trauma-informed care tailored to disadvantaged and at-risk young people.
  • Provide evidence-based therapeutic interventions, individual counselling, and case management.
  • Facilitate group work, outreach, intake assessments, and health education.
  • Collaborate in a vibrant team environment with Social Workers, Psychologists, Nurses, Aboriginal Health Workers, and Health Education Officers.

  • Tertiary qualifications in Social Work and eligibility for registration with the AASW.
  • Strong understanding of youth health and development.

Where You'll Be Working
SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.

These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.

With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).Demonstrated experience as a Social Worker in relevant clinical area (Youth Health) and an understanding of issues affecting health and well being of young people (12 - 24 years of age) and their families.Proven experience prioritising work across various disciplines - within Youth Health Services and managing stressful situations.Proven ability to work independently and collaboratively within a multi-disciplinary team.Demonstrated high level communication skills (written, verbal and computer) with demonstrated experience in developing partnerships and working effectively with a range of stakeholders.Evidence of commitment to clinical supervision and professional learning.Demonstrated understanding and application of evidence based practice, commitment to quality processes.Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
