Aged Care Case Manager & NDIS Case Manager / Team Leader (Multiple Positions)

We are seeking dedicated and compassionate Aged Care Case Manager & NDIS Case Manager / Team Leader to join our growing team, supporting NDIS and Aged Care Clients in their own home environment.
About the Role:
We are looking for an experienced full-time Aged Care Case Manager & full/part-time NDIS Case Manager / Team Leader to join our team, assisting with HomeCaring Tranmere's dedication to fostering a supportive and caring environment for both our clients and team members. The role is based at our head office in Dernancourt.
Your Key Responsibilities Include:
Aged Care Case Manager- Serve as the central point of contact for clients, their families, healthcare providers, social services, and community resources, ensuring effective coordination and communication.
- Respond to Client/Representative inquiries and address concerns promptly, maintaining high-quality customer service.
- Offer emotional support and guidance to clients and their families throughout their care journey.
- Facilitate effective communication with internal and external interdisciplinary teams and key stakeholders to ensure holistic and seamless care.
- Advocate for clients' rights and needs within the healthcare system.
- Review relevant documents and consult with clients, their representatives, health professionals, and support workers to develop individualized care plans.
- Perform comprehensive assessments and conduct care plan reviews regularly, ensuring plans reflect clients' unique needs, preferences, and goals.
- Review client budgets regularly to prevent overspending and adjust as necessary.
- Assist clients in upgrading their package level when care needs change.
- Collaborate with the team to identify and implement opportunities for continuous improvement.
- Maintain open communication with senior case managers to ensure alignment with organizational goals, policies, and procedures.
- Work collaboratively with admin team to create suitable care teams for clients.
- Adhere to established workflow procedures for Case Managers and Admin/Rostering teams, ensuring shifts are adequately covered and client satisfaction standards are met.
- Monitor participants' needs, including functional capacity, physical and mental health, and social and emotional well-being, reporting changes or concerns.
- Support onboarding of new referrals and develop individualized Care Plans & Risk Assessments in collaboration with clients, their families, and healthcare professionals.
- Maintain regular contact with internal and external departments to ensure clear and proactive communication.
- Coordinate and liaise with healthcare providers, social services, and community resources.
- Maintain ongoing and supportive relationships with clients and their families.
- Work collaboratively with the admin team to create suitable care teams for clients.
- Manage rosters of care and serve as the point of contact for support worker care teams.
- Ensure all documentation is up to date and compliant.
- Possess comprehensive understanding of NDIS standards and updated legislation.
- Have computer literacy skills and experience working with client management software systems.
About You:
- RN/EN qualification highly desirable. Relevant case management experience is preferred (Aged Care Case Manager).
- Minimum 2 years of case management or team leader experience in NDIS (NDIS Case Manager / Team Leader).
- Strong interpersonal skills to establish rapport and build trusting relationships.
- Excellent verbal and written communication skills.
- Proficiency in documentation and record-keeping.
- Ability to work collaboratively.
- Empathy, compassion, and a non-judgmental attitude.
