We are an NDIS Registered Service Provider delivering Disability Services to NDIS participants. Our vision is to enrich the lives of people with a disability. To achieve this, we follow a person-centered approach around our core values.
About the Role:
Support Coordinator - Adelaide
As a Support Coordinator, you will support people with a disability to implement their NDIS plan and link them with other service providers to ensure their support needs are met.
You will provide exceptional customer service – active listening, a proactive attitude, an attitude of Empathy towards our clients and a person-centred approach.
As an experienced Support Coordinator, you will offer assistance to individuals and their families on their NDIS journey. You will aim to increase the independence of our clients by working closely to develop their capacity, confidence, and quality of life.
You will support clients to ensure their NDIS funding is utilised appropriately and effectively. You will balance individual NDIS Plan budgets and connect clients with support services in the local area best suited to their needs.
You will work collaboratively with our clients and stakeholders to identify and implement support, resolve points of crisis, and build skills of self-management and resilience in a client's community network.
You will develop, maintain, and facilitate positive relationships with clients and their representatives, internal and external stakeholders, and key personnel within the NDIS, and other government agencies by initiating and facilitating family and provider meetings.
You must be able to maintain high attention to detail in complex report writing, case notes, and administrative requirements.
You will work collaboratively with your existing networks and other stakeholders to assist in the growth of our business programs by securing new referrals and other business development opportunities.
To win this position, you will need to prove you have:
- Proven experience building and maintaining relationships with key stakeholders
- Flexibility to attend face-to-face meetings with clients in both home and or hospital environments
- Strong communications skills both verbal and written
- Demonstrated experience working within legislative guidelines, procedures, and policies
- Strong computer skills, in Microsoft applications and Client Management Systems
- Valid Driver's License, NDIS Workers Screening Check, Working with Children Check
- Strong time management and organizational skills, with the ability to work both autonomously and in a team environment
- Demonstrated experience in Support Coordination and identifying, measuring, and reporting on client and program outcomes
- Knowledge of NDIS, Disability Act, and other relevant legislation
- Relevant Tertiary qualifications in Disability Services desirable.
Essential/ favourable attributes:
- Demonstrated existing local networks
- Work experience/ knowledge of the NDIS policies and procedures
- Outstanding knowledge of the NDIS system including the NDIS price guide
- Knowledge of the NDIS pathways to supporting a participant via supports coordination
- 12 months or more experience in a related role, working collaboratively with Allied Health to onboard participants from hospital/ home to a service provider.
