Responsibilities
  • Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  • Conduct initial and/or comprehensive nursing evaluations, determine primary care focus, develop the plan of care within state guidelines, and ensure accurate, complete, and timely documentation.
  • Regularly evaluate and re-evaluate the patient's nursing needs, at least every 60 days or as condition changes.
  • Perform comprehensive assessments at designated times and develop appropriate plans of care in collaboration with physicians.
  • Ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
  • Develop, implement, and revise the plan of care in collaboration with physicians and healthcare professionals.
Education and Experience
License Requirements
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to reliable transportation.
