ABOUT THE ROLE
We are looking for aCare Partner, Registered Nursewho is a passionate health and aged care professional looking for a new opportunity. As Care Partner you will be an integral point of contact for workers and clients to provide support for the broader community team. Join us on a permanent full-time capacity at Right at Home Sunshine Coast, located in Caloundra.

To be successful you must hold:

  • Formal tertiary qualification eg. Bachelor of Nursing or equivalent
  • Current AHPRA registration as a Registered Nurse (Mandatory)
  • National criminal history/police check
  • Valid First Aid and CPR certificate
  • Valid Blue Card

Experience:

  • Knowledge of the aged care industry/issues or Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles and the Aged Care Quality Standards – Essential
  • Previous experience in Aged Care or related discipline – Desirable
  • Easily adaptable to new software platforms

As aCare Partneryou will be responsible for:

  • Building strong and meaningful connections with clients and their family
  • Conducting comprehensive client assessments and development of care plans to support our clients to remain within their home
  • Interacting and providing professional health and clinical care advice and direction to a team of home care workers
  • Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction
  • Ensuring all practices are consistent within policy and legislative frameworks
  • Maintaining up to date client records and information
