Central Coast Local Health District (CCLHD) are looking for caring and compassionate Health Clinician Level 4 for Older Peoples Mental Health Service. The successful candidate will possess specialist knowledge regarding mental health specific assessments, clinical treatments, and be able to co-ordinate care for individuals with complex needs.
This role is suitable for the following Grades:Social Worker Level 4
Occupational Therapist Level 4
Senior Psychologist
Clinical Psychologist
Clinical Midwife Consultant Grade 2
Clinical Nurse Consultant Grade 2
- You have the ability to meet the Essential Criteria for this role with extensive experience in the mental health assessment, treatment, and management of people over the age of 65 years.
- You possess high-level clinical leadership capacity and clinical supervision skills with commitment to a consumer-focused recovery-oriented approach and consumer collaboration in care planning.
- You have experience in project work or service development activities that involve planning, developing, implementing, reviewing and evaluating new initiatives within a health service context.
As the health clinician level 4 you will enhance capacity for specialist Older Persons Mental Health in-reach into residential aged care facilities (RACF) by developing, facilitating, implementing, and evaluating care management plans for the Specialist Dementia Care Program (SDCP) residents with behavioural and psychological symptoms of dementia (BPSD) and emerging and chronic high-risk mental health conditions.
You will provide timely consultancy, clinical coordination, advice, and assessment where appropriate to residents in the Specialist Dementia Care Program who are experiencing acute or complex mental health issues.
You will promote safe and timely transitions to and from mainstream residential aged care and increase access to OPMH services for aged care residents and assist in preventing unnecessary hospital presentations and admissions.
