Position DescriptionThe Assessment Officer (AO) works with the aged care assessment service multi-disciplinary team to provide non-clinical aged care assessments independently in a home or community setting, for older adult clients. The AO uses the Integrated Assessment Tool (IAT) and My Aged Care (MAC) systems to identify care needs and eligibility, supporting access to appropriate care including Commonwealth Government subsidised aged care entry level services.
Rockingham General Hospital and Murray District Hospital provide inpatient, outpatient, palliative and allied health services. SMHS aims to provide high quality health care across its network.
