Care Coordinator - Aged Care

As our Clinical Care Partner (Registered Nurse), you'll be working alongside clients, families, and service providers to co-design care that is safe, effective, and deeply personalised. This role involves working with older, vulnerable clients who may have a history of hardship, homelessness, mental health challenges, or other complex life experiences. You'll blend your clinical expertise with a people-first, trauma-informed approach, enabling older clients to live life on their terms, while ensuring services meet all funding and quality standards.

Your day on the job
  • Complete clinical, care, and risk assessments to develop care plans that support the client's individual goals.
  • Establish care and service plans in line with Commonwealth Government Support at Home guidelines and the client's package budget.
  • Make referrals to allied health professionals and service providers to ensure agreed care plans are fully delivered.
  • Ensure all care and service plans are accurately budgeted and aligned with the client's package parameters.
  • Conduct timely reviews of care plans following changes in the client's condition, hospitalisations, or other clinical events.

  • AHPRA registration as a Registered Nurse within the aged care, community services or health sector.
  • People skills that shine - you enjoy building relationships and thrive in a collaborative, multidisciplinary team
  • Proven experience in delivering person-centered care and support, and ability to work within services that promote client wellbeing
  • Deep understanding of the challenges, needs, and goals of older people, their families, and carers
  • Experience working with diverse and vulnerable populations, including clients, allied health professionals, and service partners
  • Knowledge of the Support at Home Program, Home Care Packages (HCP), and aged care funding systems
  • Tech-savvy – confident using Microsoft Office and client management databases

About Us

For 60 years, St Bart's has supported people experiencing homelessness and severe and persistent mental health issues as they rebuild their lives. We started as a small homeless shelter for men and have grown into a progressive and inclusive organisation providing support to hundreds of Western Australian men, women and families across four key service areas. Our goal is to provide safe, supported accommodation and services to people who need it the most, regardless of background, age, gender or ethnicity.
