Position Description – Support Worker

Position Title: Support worker

Reporting To: Director Operations

We are looking for our next intake of rock star disability support workers looking to make a difference in the lives of NDIS participants.

About Us

INNOVATIVE CARE SOLUTIONS is an NDIS provider working with people with disability to make it easier for them to access services they need under the NDIS support service. We aim to create an environment of service provision which creates a smoother journey for all involved, including our staff and participants.

In order to ensure that we are meeting the needs of people with disabilities and our participants, we will be engaged with healthcare professionals, local councils, community groups and individuals. We will also engage with other NDIS providers in order to share, learn and collaborate on opportunities that enable us to strengthen the support base for people who need our services.

Position Purpose

The Support Worker will develop and monitor their individual recovery journey and support and assist them to achieve their individual goals by developing self-efficacy, increasing wellness, community participation, self-reliance, and overall confidence, potential, and happiness.

Key Accountabilities

  1. Comprehensive knowledge of the NDIS support items under the NDIS Guidelines.
  2. Direct involvement, assessment, and support of clients ensuring policies and procedures are adhered to.
  3. Develop and maintain positive working relationships and linkages with community agencies, clinical services, and all other relevant providers to ensure effective communication, referrals, and care coordination can occur.
  4. Working one-on-one or in a group setting supporting clients to make their own decisions, overcome barriers and strive towards achieving their goals.
  5. Advocate on behalf of clients to access services within the community.
  6. Monitoring and reporting including monitoring participant referrals, progress against NDIS Goal Plans, data collection, and reporting where required.
  7. Mandatory administration tasks including data entry into the client management system.
  8. Undertake ongoing professional development as required.
  9. In addition to the above responsibilities, you are required to also perform any other reasonable duties as directed from time to time.

Key Competencies Knowledge

  • A current and valid Working with Children Clearance
  • Tertiary qualifications in a relevant discipline
  • Demonstrated experience in supporting individuals recovering from severe and persistent mental illness
  • Excellent networking, interpersonal and written communication skills
  • Current Drivers License, willingness to travel and access to a comprehensively insured motor vehicle

Qualifications:

Tertiary Qualification in a relevant discipline such as Social and Community Services, Disability Services with relevant experience in a similar service.
