About the Program

Join one of our newest programs within our rapidly growing service portfolio Our Treatment services in Residential Aged Care Facilities aim to enhance the quality of life for older people through improved access to primary mental health services, using a person-centred stepped care approach.

This program provides in-reach healthcare services to residents in aged care facilities, targeting those diagnosed with or at risk of mental illness. It's a rewarding role where you can make a significant, lasting impact on residents' quality of life.

The program is led by a compassionate, curious, and non-judgmental leader passionate about supporting staff and fostering innovation.

Your Opportunity

As a Residential Aged Care Group Facilitator, you will conduct brief intake and assessment processes to determine residents' suitability for group participation or refer higher support needs cases.

This autonomous role involves regular regional travel to aged care facilities, where you will facilitate 2-3 hour groups on topics like mental health understanding, reframing strategies, mindfulness, and social connectedness. These groups are based on acceptance and commitment therapy, supporting residents to manage their mental wellbeing independently and improve their social, emotional, and psychological health.

Responsibilities
  • Facilitate training and group sessions confidently.
  • Engage with local providers, building positive relationships and a local presence.
  • Collect outcome measures and provide feedback for program improvement.
  • Build rapport with participants and connect them to appropriate resources.
  • Represent LLW and the program during facility visits.

You will be supported by an experienced Team Leader and an Allied Health Practitioner - Social Worker, with additional support from our multidisciplinary team.

Role Details

This is a full-time ongoing position, working Monday to Friday. The salary ranges between $88,109 and $94,828 plus superannuation and benefits. Part-time arrangements and flexible working options are available for suitable candidates.

Ideal Candidate

This role suits individuals with a nursing or aged care background, with experience in mental health or related fields. No ongoing counseling is required, making it suitable for new graduates as well.

  • Knowledge of mental health conditions in older populations.
  • Australian Driver's Licence.
  • Relevant tertiary qualifications or working towards them (e.g., in Social Work, Psychology, Health, or Mental Health Nursing).
